People encounter an ad for The Presence Shift on Facebook or Instagram and tap its call-to-action to open the form. The screenshots below show the contact-information step, the SMS consent step, and the after-submission confirmation preview.
These are screenshots from Meta’s form editor and customer preview. This evidence page does not collect information or enroll visitors in SMS. The program is pre-launch.
1. Phone number on the same Meta form
The contact-information step contains Email, Name, and Phone number. The phone field and the SMS consent choices appear on separate steps of this same multi-step form. Entering a phone number does not, by itself, select either SMS consent option.
2. Two independent, optional SMS choices
The customer preview shows both SMS checkboxes unchecked. Users may select either option, both, or neither. The requested-video choice does not authorize recurring marketing texts. Both choices identify the same operator: One Perfect, Inc., which operates The Presence Shift.
